3 Applicant Assessment Forms and accompanying letters of assessment from academic and/or professional assessors who have known you for at least one year in a capacity enabling him/her to judge your ability and performance (ie., teacher, trainer, advisor, employer, etc.).

Formsshould be filled out and accompanying letters should be written individually by each assessor.

Each assessor's form and letter should be submitted in an envelope signed and sealed by the assessor.

Laboratory results should be sent for standard, blood tests, blood tests for STD's (including HIV), urinanalysis, x-rays.
